Signatures — the design cannot complete without both
Independent under SS 204.03.02 — not an employee of the contractor, an explosives manufacturer or a distributor.
Printed name · company · certificate number recorded
The certificate number ties the plan to a person legally permitted to execute it. Checked against the qualification register on signing.
A change to previously used drilling or blasting methods is a new version, needing its own signature and a minimum of one day's notice.
Pre-blast survey — due at least one day before
| Kind | Identity | Distance | In proximity zone | Existing condition |
|---|---|---|---|---|
| Medical clinic | Vernon walk-in, 35th St | 280 m | yes — 300 m | recorded, 4 photographs |
| Occupied structure | 3412 35th St | 96 m | yes — 150 m | recorded, 6 photographs |
| Occupied structure | 3418 35th St | 104 m | yes | recorded |
| Well | Private, lot 22 | 88 m | surveyable, not notifiable | recorded |
| Utility | Buried gas main | 40 m | surveyable, not notifiable | located and marked |
Survey method recorded, and confirmed acceptable to the contractor's insurance company as SS 204.04.08 requires. The same structure list drives the 48-hour occupant notices — one geometry, two uses.
Close-proximity written plan — in force
| Author qualification | ≥5 years urban blasting · EGBC member in good standing |
| PPV limit | 3.0 mm/s |
